SS1 - Philanthropy-Powered Patient Engagement at Scale - Solution Spotlight

Community Health Centers face the ongoing challenge of engaging hard-to-reach patients, closing care gaps, and aligning with value-based care incentives. CareMessage has partnered with FQHCs for 12 years to expand access and achieve clinical outcomes by combining patient-first tools, patented virtual sequential health coaching programs, and partnerships that scale impact across networks.

This session will highlight how health centers and associations are building scalable patient engagement strategies. We will share best practices from our centers that generated reductions in no-shows and additional quality reimbursements, as well as findings from third party randomized controlled trials, validating the effectiveness and role of CareMessage’s coaching solutions in supporting patients with chronic disease management and preventive care.

Two association-led initiatives will further illustrate the power of scale: a centralized patient engagement strategy that reaches unseen patients for lead screening, and a multi-year network program engaging hundreds of thousands of patients to increase annual wellness visits, improve preventive screenings and chronic care management. Together, these examples demonstrate how combining technology and networks creates one-to-many referral pathways, reduces duplication of outreach, and generates measurable improvements in UDS-aligned outcomes.

This interactive session will conclude with practical insights on how to design sustainable, scalable engagement strategies that leverage the right tools, validated interventions, and network partnerships to drive health equity and impact at scale.
