Session Description: This session will highlight partnership models that leverage complementary strengths across organizations to build health center capacity, improve health outcomes, and provide community-informed care. By highlighting T/TA approaches centered on improving health outcomes, patient-level data collection on non-medical factors of health needs, limited English proficiency, and community-based workforce development, the session underscores how health centers can enhance quality of care while building trust in communities.
Learning Objectives:
Describe effective T/TA strategies that address non-medical factors of health, Serving Individuals with Limited English Proficiency (LEP), and workforce integration.
Identify opportunities for partnership among PCAs, HCCNs, and NTTAPs to enhance health center capacity and strengthen workforce.
Apply educational and training opportunities for patient-level data collection strategies to improve patient engagement and health outcomes in health center populations.
